You can use this form to register for sacrament preparation at Holy Name Parish. You will be contacted to confirm your registration.
If your child was baptized at Holy Name Parish, please complete the Baptism information section - but we will not require a copy of the Baptism certificate. If he or she was baptized elsewhere, we will need a copy of the Baptism Certificate. The copy can be faxed to us at 249-9952 or brought with you on the first day of class.
A $20.00 registration fee covering the cost of books is payable in cash or cheque to Holy Name Parish. Please bring the payment with you on the first day of class.
Family Commitment By enrolling your child in preparation for a sacrament, please realize that it is your job to learn as much as you can about this sacrament in order to help and support the efforts of the parish in your home. We ask you to continue to reinforce the Catholic tradition as best you can within your family.
